CMS announces Medicare contractor annual update of ICD-9-CM
On July 29, CMS issued a transmittal announcing its annual reminder to the Medicare contractors of the ICD-9-CM update.
Effective date: October 1, 2008
Implementation date: October 6, 2008

CMS maintains existing National Coverage Decision (NCD) for microvolt t-wave alternans (MTWA)
On July 25, CMS issued a transmittal announcing that it will not change its existing NCD for MTWA.

CMS issues National Coverage Decision (NCD) for home prothrombin time INR (PT/INR) monitoring for anticoagulation management
On July 25, CMS issued a transmittal announcing an NCD for the use of home PT/INR monitoring. CMS will cover the use of home PT/INR monitoring for chronic, oral anticoagulation management for patients with mechanical heart valves, chronic atrial fibrillation, or venous thromboembolism (inclusive of deep venous thrombosis and pulmonary embolism) on warfarin.
Effective date: March 19, 2008

CMS issues update to screening DNA stool test for colorectal cancer
On July 25, CMS issued a transmittal announcing that it will not expand the colorectal cancer screening benefit to include coverage of PreGen-Plus, a commercially available screening DNA stool test.

CMS issues correction to National Coverage Determinations manual section on continuous positive airway pressure (CPAP) therapy for obstructive sleep apnea (OSA)
On July 25, CMS issued a transmittal that corrects a previous transmittal updating the Medicare National Coverage Determinations manual to expand coverage for CPAP therapy for OSA to include a positive diagnosis of OSA made using a home sleep test under specified criteria.
Effective date: March 13, 2008
Implementation date: August 4, 2008

CMS issues correction to guidance on billing for new hemophilia clotting factor code
On July 25, CMS issued a transmittal that corrects previous guidance on billing for HCPCS code Q4096 (Injection, Von Willebrand Factor Complex, Human, Ristocetin Cofactor) furnished to inpatients.
Effective date: April 1, 2008
Implementation date: January 5, 2009

CMS issues correction to guidance on billing for the technical component of pathology services furnished to hospital patients
On July 25, CMS issued a transmittal that corrects previous guidance on the implementation of a regulation prohibiting non-hospital laboratories from billing Medicare directly for the technical component of pathology services furnished to hospital patients.
Effective date: July 1, 2008
