'Present on admission' rule makes documentation vital

Medicare doesn't want to pay hospitals to treat problems patients developed due to their stay at a medical facility. The focus on determining what medical conditions are present at the time a patient is admitted to a facility could pose major implications for hospital financial reimbursements, institutions' reputations, and the day-to-day operations of compliance and auditing professionals.

A new policy

The change originates with a July 20 transmittal by the CMS (Transmittal 289, Change Request 5679).
