New tools to support care transition

The National Transitions of Care Coalition (NTOCC) is to pilot a set of tools, including checklists, patient medicine lists, and standard forms, designed to improve the coordination of healthcare.
The organization, which includes representatives from case management, as well as pharmaceutical and consumer groups, says the tools will help in the transitioning of patients from one level of care to another.
"The development of these critically needed tools represents an important step in solving transitional problems," said Cheri Lattimer, RN, BSN, executive director of the Case Management Society of America, in a press release. "In just under a year, the 27 NTOCC advisory task force organizations have accomplished a tremendous amount by identifying the issues, gaps, and barriers that create transitional challenges, and agreeing to work together to address an industrywide problem."
The tools are currently scheduled for an early 2008 launch.
