BlueCross implements new care management technology

By the end of this year, BlueCross BlueShield of South Carolina intends to execute a new care management platform that will electronically integrate personal health records with medical care plans. The platform is a single IT application available in real time to BlueCross members and their healthcare providers, incorporating each patient's personal health record with pertinent case management, disease management, and maternity management data. As a result, information about any chronic diseases or high-risk conditions will be immediately available in every patient's personal health records.
BlueCross has partnered with ParadigmHealth, a disease management service provider designed to improve quality patient care and lower costs, to develop the new technology based on the individual needs of patients, especially those with chronic conditions. Furthermore, the two companies plan to make the integrated care management platform available to other insurance companies.
