Document helps low-literate patients understand advance directives

In an effort to help all patients make their healthcare preferences known, the Institute for Healthcare Advancement (IHA) recently released an advance directive form written at a fifth-grade reading level. Most advance directives are written at a post-graduate reading level, according to Rebecca Sudore, M.D., who developed the document with help from staff at San Francisco General Hospital and the San Francisco Department of Public Health. For those with limited reading skills, the complexity of most directives makes it intimidating and difficult to make their wishes known, said Sudore.
