Cardiovascular discharge checklist reduces LOS and morbidity

A discharge checklist reduced readmission and morbidity rates of heart failure patients, according to a recent study in the Annals of Internal Medicine. Previous studies found that when heart failure patients were discharged only 55% were given life-saving angiotensis-converting enzyme (ACE) inhibitors. The checklist was part of a discharge medication program that ensured patients were receiving medications, such as ACE inhibitors, upon discharge. After staff began using the checklist death rates for heart failure patients dropped 23% and the readmission rate for these patients fell 9%.
