Elderly heart failure patients receiving specialized nursing care less likely to return to hospital

When elderly heart failure patients receive specialized nursing care in the hospital and at home after discharge, they are less likely to be readmitted-resulting in a 38 percent savings in Medicare costs. Researchers came to this conclusion following a study conducted in six Philadelphia hospitals. Advanced practice nurses coordinated the patients' care with their physicians, pharmacists, social workers, and RNs. This level of care cost almost twice as much as routine care provided to elderly heart failure patients, but the increase in cost was offset by the savings seen from fewer hospital readmissions. Elders with heart failure have the highest rate of hospitalization and cost $24 billion each year.
