Access Community Health in Chicago equipped its case managers with wireless Tablet PCs that allow them to access the organization's database remotely and refer people to medical care or other help using Cogent Equicare Case Manager.
Access operates 42 health centers and one dental health center, servicing nearly 150,000 patients annually throughout Chicago.
According to a report in the Chicago Sun-Times, the system has reduced referral time from hours to less than 10 minutes.
Case managers enter the person's vital information into the Tablet PC, answer 22 assessment questions and then refer the person to the proper agency. The agency then receives the electronic referral, which staff members either accept or decline. The case manager receives the response and either sends the person to the right place or tries another agency.
