Ten chronic care programs to launch by year's end

Medicare plans to launch 10 chronic care programs throughout the country by year's end. The programs will monitor those with chronic diseases such as obstructive pulmonary disease, congestive heart failure, and diabetes-and help coordinate their care.

The Voluntary Chronic Care Improvement Program will assist between 150,000 and 300,000 Medicare beneficiaries. These patients will receive support and information about other Medicare benefits that will reduce their risk of complications, and program staff will coordinate care with the patient's physician and other physicians they may see.
 
The 10 pilot programs will last for three years and serve as the foundation for the national program.
