Preventing improper discharge in pressure ulcer cases

Pressure ulcers have rapidly become the number one legal issue for hospitals. The first step in preventing pressure ulcers is to complete a thorough skin assessment on all residents. Skin assessments should include an examination of the entire skin surface, nutritional status, mobility, positioning, continence, diagnoses, infections, and any history of skin problems.
