Case managers key to Navy Hospital diabetes education

The Navy Hospital in Pensacola, FL, has started a diabetes education program. Called "DM drop-ins," patients come to a large conference room at the Naval Hospital once a month. Table "stations" are set up to give patients a one-on-one review of their disease management, from specialists they would have to wait months to see. There's a nutrition station, a pharmacist, a foot station so patients can be checked for extremity damage, and a case management station. Here, the case managers talk about transportation issues, access to care, and the person's progress on the care plan.

On February 10, the hospital kicked off another drop-in session for congestive heart failure patients.
