Communication among physicians, nurses, and allied health professionals is generally best handled in person, and one on one. Talking to your colleagues one on one about case management activities generates much more intrinsic value than the written word. And when documenting your conversations, limit it to a few words on the integrated program notes specifying distinct case management activities.
