Pharmacist initiative drops readmission rates

Can your pharmacists help reduce readmissions at your organization? A new study presented at the American College of Clinical Pharmacy 2015 Global Conference says yes.

Under an initiative designed to help people better understand and take their medications properly, pharmacists from the St. Louis College of Pharmacy started contacting patients within 48 hours of discharge. Patients involved in the initiative were not only less likely to see a doctor in the two weeks after they left the hospital, but those who did see a doctor were 40% less likely to be readmitted when compared to patients who didn’t work with a pharmacist.
