College students help cut readmissions at Ohio hospital

An Ohio hospital has slashed its readmission rate thanks to an innovative program that uses college students as health coaches.

The students visit each of their two assigned patients weekly to assess resources and help connect them with services in exchange for volunteer credit hours. The program that began two years ago reduced emergency department usage by 26% and cut the readmission rate by more than half.
