New tool may help predict readmissions

An assistant medical professor at the University of Wisconsin-Madison has created a tool that she says hospital can use to predict which seniors are at highest risk for a hospital readmission.

Amy Kind developed the tool, which looks at the neighborhood the patient lives in to predict their likelihood of boomeranging back to the hospital after discharge. In addition to serving as an assistant professor at the university, Kind is a geriatrics specialist at William S. Middleton Memorial Veterans Hospital in Madison, Wisconsin. Seniors who live in the 15% “most disadvantaged” neighborhoods in the U.S. are most likely to return.
