Tip: Align case managers with social workers

For models of practice that separate case management and social work, the best advice is to make the social worker your best friend. Working together is often necessary—for example, in cases of suspected elder abuse or elders found living in squalor. Because social workers possess a wealth of knowledge about community resources, shelters, polysubstance abuse programs, and family dynamics, they are important partners in discharge planning.
