Tip: Case management for children with special needs

Children with complex medical conditions and technology dependence are frequent visitors to pediatric emergency departments (ED). Their care often involves collaboration by multiple hospital specialists and coordination with community and home care providers. Their visits to the ED provide an opportunity for case managers to learn how families are managing at home and whether problems or concerns that a case manager may be able to address exist. Case managers can advocate for team responsiveness to families’ special concerns and assist with coordination of care during ED visits. They also may be able to suggest additional support services available for these families, such as:
- Respite services
- Home asthma management programs
- Ongoing case management services
