News: HHS launches Community-Based Care Transitions Program

The Partnership for Patients is a national partnership that aims to save 60,000 lives by stopping millions of preventable injuries and complications in patient care over the next three years, according to Health and Human Services Secretary Kathleen Sebelius.
The Community-Based Care Transitions Program (CCTP) demonstration project, which will fund efforts to improve transitions from hospitals and reduce readmissions, is a major component of the Partnership for Patients.
HHS is accepting applications for hospitals that want to participate in the CCTP. Community-based organizations and acute care hospitals that partner with community-based organizations may submit applications for a share of the $500 million in CCTP funding. CMS is accepting applications on a rolling basis and will award resources on an ongoing basis as funding allows.
