Tip: Steps in the discharge planning process

Both discharge planning and transitional planning apply a process whereby a systematic approach facilitates the transition of the patient from one level of care to another, whether within the acute care continuum or beyond. Follow these steps to complete the process:

  • Plan the patient stay from door to door and beyond
  • Collaboratively determine the level of care with the interdisciplinary team
  • Connect the patient to continuing care services
  • Transition the patient to the next level of care
