Discharge planning, which is integral to providing continuity of care, is a dynamic process in which care providers assess patient needs, identify the resources necessary to meet those needs, establish and implement plans to meet those needs, and move the patient to the most appropriate level of care. The steps are what make this process dynamic—they sometimes occur in order; sometimes overlap, and sometimes move back and forth.

The process of discharge planning is composed of six steps:
  1. Casefinding and screening
  2. Evaluation of the patient’s continuity-of-care needs
  3. Assessment of resources
  4. Problem identification
  5. Planning
  6. Implementation
