A patient can come into your facility and get the best care possible, but if you don’t have a strong discharge plan, the plan of care can unravel or, worse, you may find yourself looking at readmissions.

University Children’s Hospital at UC Irvine Medical Center in Orange, CA, realized that a little extra support could go a long way when it implemented its Patient Discharge Callback Program three years ago.
