As facilitators and coordinators of patient care services, case managers are in the best position to identify potentially avoidable days. Avoidable days are caused by delays that occur across the continuum of care, whether in the sphere of influence of physicians, case management/discharge planning, patient and family, internal nursing/ancillary services, or external providers.

Organizations can identify causes of avoidable days by examining their avoidable day data and looking for trends. Each time a potential or actual delay is identified, it should be:

  • Forwarded to the manager or director of the service for analysis, intervention, and future prevention
  • Trended for frequency to identify processes that can minimize these delays

Identifying and trending of avoidable days and sharing the information that these issues cause patient flow barriers will help mitigate many delay and flow problems. This will ultimately improve the safety, quality, and cost of patient healthcare.
