What role does the case manager play in coordinating and implementing core measures?

No longer just a Joint Commission requirement, core measures have hit everyone’s radar and have become the hallmark of pay-for-performance initiatives. As hospitals face the dilemma of how to meet expectations set bytheir core measures, the question remains as to who is in the best position to ensure that the core measures are carried out. It seems like a natural fit for case managers to make sure core measures are implemented, because in the past 10 years, case managers have emerged as the individuals whofollow a patient through the entire continuum of care. This puts them in an ideal situation to work with physicians and nurses to ensure that core measures are accomplished and the most accurate documentation is in the patient’s medical record.
