Taking an interdisciplinary approach to attacking avoidable days

As facilitators and coordinators of patient care services, case managers are in the best position to identify potentially avoidable days. Avoidable days are caused by delays that occur across the continuum of care, whether in the sphere of influence of physicians, case management/discharge planning, patient and family, internal nursing/ancillary services, or external providers.
With an increasing focus on measuring the efficiency and effectiveness of healthcare services, it makes good sense to involve the entire interdisciplinary care team and medical staff in identifying and correcting any delays in the progression of patient care. These interruptions in patient flow reduce the safety and quality of patient care, affect patient satisfaction, and increase costs to patients, insurers, and hospitals.
