Spectrum Health in Grand Rapids, MI, has an award-winning care management program, which combines the expertise of social work case managers, nurse case managers, and utilization management specialists to create a uniquely collaborative program. Before developing this new program, Spectrum’s case management department was primarily social work driven.
In order to create this new program, Spectrum Health pulled together a large interdisciplinary group from across the hospital system to analyze opportunities to improve the system’s discharge planning process. It recognized an opportunity to create an interdisciplinary approach to patient care.
Nan Hunt, LMSW, CMAC, says it was important to ensure that the purpose of the team was not to have parties relinquish expertise, but step into new expectations that focused on collaboration. She says Spectrum was able to do that by ensuring no team member is dominant. Instead, each team is enhanced by incorporating three distinct points of view and expertise.
