Medical error leads center to reexamine procedures

The death of a three-year-old boy due to an overdose of a substance used for testing growth hormone deficiency has caused the facility to reexamine its existing procedures, reports the Miami Herald.

Sebastian Ferrero died after he received a dose 10 times larger than intended while undergoing tests at Shands HealthCare facility at the University of Florida in Gainesville, FL.

Following the incident the facility put a moratorium on all similar procedures at its outpatient clinics, as well as required retraining for all personnel. Shands will also add a double-sign-off system to prevent further errors.
