Patient perception a problem with medication reconciliation

A study by Northwestern University demonstrates a need for future research to address patients' inability to remember the names or usage of their medications.

Ongoing problems such as low health literacy and refilling of old prescriptions after they have ceased to be necessary are causing a disconnect between physicians and patients in the area of medication reconciliation.
