Planning, relationship-building help to comply with NPSG targeting patient handoffs

Improving patient handoffs is among the National Patient Safety Goals for 2007. To improve on this focus area, Baltimore's Union Memorial Hospital has used a combination of building strong working relationships with referring facilities and special training for staff to improve handoffs of elderly patients.
