Pharmacist indicted after lethal chemotherapy dose

A former Cleveland-based pharmacist faces charges of involuntary manslaughter and reckless homicide following the death of a two-year-old girl who died after receiving an incorrect dose of chemotherapy, the Associated Press reports.

The pharmacist, Eric Cropp, did not catch an error in mixing the chemotherapy solution, which contained 23.4% sodium chloride solution instead of a 1% solution, before the dose was administered. The solution was mixed by a pharmacy technician.

According to Cuyahoga, OH, assistant district attorney Paul Soucie, the pharmacist is being charged because there were a number of warning signs he should have caught.
