Joint Commission releases updated Sentinel Events statistics

Accreditation Connection, March 19, 2007

The number of reported Sentinel Events was down in 2006 compared to the last two years, according to statistics released last week by The Joint Commission.

Healthcare facilities self-reported 344 Sentinel Events in 2006, down from 367 in 2005 and 347 in 2004. That number represented 65% of all reported Sentinel Events, the highest percentage since 2002.

Sentinel Events reported by someone other than the facility also dropped in 2006, down to 182 from 215 in 2005 and 214 in 2004. A record number of Sentinel Events reported was set in 2005, so a drop could have been expected.

The Joint Commission started collecting Sentinel Event data in 1995 and has reviewed 4,074 total Sentinel Events in the time frame, 2,766 of which came from hospitals. Wrong-site surgery is the most frequently reported Sentinel Event, with 532 total reports since 1995, followed closely by 522 reports of suicide. A patient's death was a result of 3,021 of the reported Sentinel Events.
