Med rec, med error efforts in Mass in the spotlight

The May issue of Health Executive features a story about the Massachusetts Coalition for the Prevention of Medical Errors (Mass Coalition) and its efforts on medication reconciliation (JCAHO National Patient Safety Goal #8).

Also concerning medication errors, an Accreditation Connection reader pointed out that last week's edition included a link to an Indiana newspaper article about states requiring medical mistake reporting that didn't include activities going on in Massachusetts.

The reader points out that the Massachusetts Patient Care Assessment Program was legislated into existence in 1987. It requires not only reporting of major incidents, as defined by the legislation (e.g., maternal deaths related to delivery, death from ambulatory procedures, wrong organ/extremity or body part surgery, and deaths not ordinarily expected as a result of patient's condition on presentation), but also semi-annual and annual reports on occurrences. The reports are self defined by each hospital.

The reader says it's a comprehensive system overseen by the Massachusetts Board of Registration and is mandated by law to have its provisions included in every hospital medical staff bylaws in the Commonwealth of Massachusetts.
