JCAHO revises look-alike/sound-alike list

The JCAHO has updated its look-alike/sound-alike drug list for the first time in several years.

The 13-page document affects the hospital, critical-access hopsital, ambulatory, office-based surgery, behavioral healthcare, assisted living, disease-specific care, home care, and long-term care accreditation programs.

Adhering to the list complies with National Patient Safety Goal #3c: Identify and, at a minimum, annually review a list of look-alike/sound-alike drugs used by the organization, and take action to prevent errors involving the interchange of these drugs.
