Updated sentinel event statistics released

The JCAHO updated and released its sentinel event statistics. The most recent data includes reported events as of June 30. Since the voluntary sentinel event database began in January 1995, the JCAHO has received 3,197 reports of sentinel events.
The 10 most frequently reported sentinel events include the following:
· Patient suicide-430
· Operative/postoperative complication-404
· Wrong-site surgery-394
· Medication error-345
· Delay in treatment-243
· Patient fall-161
· Patient death or injury in restraints-129
· Assault, rape, or homicide-110
· Perinatal death/loss of function-97
· Transfusion error-90
