What does the JCAHO require as part of the medication reconciliation process?

Maureen Gibbs, RN, BSN, nurse manager at Milford (MA) Regional Medical Center, says JCAHO requires that hospitals have a process in place to obtain an accurate list of current medications from a patient and that the process include a comparison of the hospital's medications to the patient's home medication list. The medication information must be communicated to the next provider when the hospital refers or transfers a patient to another setting, practitioner, or level of care inside or outside of the organization.
