Organizations don't need to add to "do-not-use" list

The JCAHO lifted the requirement in National Patient Safety Goal 2b that hospitals must add three organization-specific "do-not-use" abbreviations to their list, according to the June JCAHOnline.

For accreditation purposes, the JCAHO's "do-not-use" list will be sufficient. The list applies to all orders and medication-related documentation that are handwritten, including free-text computer entry and preprinted forms.

The inability to stop utilizing "do-not-use" abbreviations in medication orders remains one of the most frequent noncompliance issues in JCAHO surveys. Twenty-seven percent of hospitals were noncompliant in this area in 2004, says the accreditor.
