Monitor medications patients bring with them to the hospital

The Joint Commission on Accreditation of Healthcare Organizations' (JCAHO) medication standard MM.2.40 requires organizations to create a process to safely address medications brought into the organization by patients or their families, and MM.4.80 requires facilities to safely handle medications returned to the pharmacy.
Make sure that you have a complete and competent policy to help your hospital comply with these standards. When a patient brings his or her medication into the hospital, most facilities identify each medication, as patients often put different pills in different containers.
"We make sure that we visually inspect the product for integrity, and if it's fine, we put it back in stock and give the patient a credit," says Donna Soflin, director of pharmacy at Tri-County Hospital in Lexington, NE.
