NPSF releases root cause analysis guidelines

A new report from The National Patient Safety Foundations (NPSF) highlights new guidelines developed to help healthcare organizations use root cause analysis (RCA) to learn about how errors occurred.

The report, RCA2: Improving Root Cause Analyses and Actions to Prevent Harm, was written by a panel of experts that examined RCA best practices and developed guidelines to standardize and improve the process. James Bagian, MD, PE, and Doug Bonacum, CSP, CPPS, who co-chaired the panel, will discuss the NPSF report in a free webcast on July 15 (Register for the webcast here.).
