Standard readiness: Targeting patient provider communications

Documenting care

 

Don't forget to discuss the following when documenting care:

  • What documents realistically need translated or revised? Consider the following; informed consent documentation, benefit denial or eligibility information, applications to receive services and information regarding the availability of translation services.
  • How is the patient’s “designated” family or “support person” engaged? Are their preferences being met? A designated support person for education, decision making or treatment processes may not be the surrogate decision maker.
  • Were staff trained on your practices and was the training adequate?
