Medication Reconciliation update

The Joint Commission announced this week the results of a July 27 discussion of the results of its recent field review of its medication reconcilation National Patient Safety Goal (Goal 8).
The Standards and Survey Procedures Committee examined the results, which indicated by a vast majority of comments that while the goal itself targeted a significant issue in patient safety, the actual implementation of said goal raised many concerns. Specifically, comments indicated that some of aspects of medication reconciliation were actually behond an organization’s control (i.e., the patient reliabily reporting current medications).
A revised requirements will present a revised requirement in October based with changes based upon the field review comments, with the an implementation date of July 2011.
At this time, NPSG 8 findings will not factor into accreditation decisions or cause Requirements for Improvements.
