CMS pilots program to prevent readmissions

The Centers for Medicare & Medicaid Services (CMS) announced the names of 14 communities nationwide that will be participating in the “Care Transitions Project,” a program aimed at eliminating unnecessary hospital readmissions.
The goal of the project is to promote seamless transitions between locations and caregivers in an attempt to lower readmissions and improve the quality of healthcare provided community-wide.
