One in five Medicare patients readmitted within month of discharge

Roughly 20% of Medicare patients are readmitted to the hospital within 30 days of discharge, a new study by the New England Journal of Medicine reports.

Researchers looked at 11.9 million patients and found that a third of those patients were readmitted within three months of discharge. The most common ailments: heart failure and pneumonia. Half of the patients readmitted did not receive outpatient follow-up visits after the initial hospitalization.
