Boston hospital uses metrics to improve medication reconciliation

Beth Israel Deaconess Medical Center in Boston, MA has focused on looking at process improvement and workflow, John Halamka, MD, CIO, explains in a recent blog post. This includes three major workflows—emergency department, inpatient, and outpatient. The hospital makes sure a complete medication list is taken at every transition, and that list goes on to the next provider of care.

Halamka points out that rigorous monitoring of this process is key to its success. The facility requires that clinics with less than 30 visit a day reviews every case for appropriate medication reconciliation. Clinics with 30 to 100 cases requires 30 of those cases to be reviewed. Clinics with more than 100 cases are required to review 50 cases.
