OIG releases reports on adverse events

The US Department of Health and Human Services Office of the Inspector General (OIG) has released a pair of reports looking at adverse events in hospitals.
The reports, Adverse Events in Hospitals: Overview of Key Issues and Adverse Events in Hospitals: State Reporting Systems, look at methods to reduce incidence of adverse events. The former conducted interviews and analyses to identify critical areas for understanding adverse events in hospitals. The latter is intended to inform decision makers on so-called “never events” and other situations resulting in patient harm while under the care of a healthcare facility.
