Inconsistency exists in adverse event reporting

Accreditation Connection, December 11, 2008

A new study shows that although 94% of U.S. hospitals have centralized systems for gathering reports of adverse events, a scant 21% actually distribute and consider adverse event summary reports, Medical News Today reports.

The study, funded by the Agency for Healthcare Research and Quality, found that only 32% of hospitals have developed environments supportive of anonymous reporting of adverse events. Ninety-six percent of these events were reported by the nursing staff, while only 13% of the facilities surveyed have involvement across the board from staffers in reporting adverse events.
