Investigation finds verbal check at heart of surgical error

Operating room staff skipped a key verbal safety check prior to performing surgery on the wrong side of a patient’s body at Beth Israel Deaconess Medical Center, the Boston Herald reports.
According to a 12-page report released following a state investigation, a surgical site marking error also occurred following the failure of the verbal check. However, investigators stated that Beth Israel has taken immediate corrective action to prevent the error from repeating.
