Effect of the 2009 NSPGs on medication reconciliation

Medication reconciliation continues to be a challenging goal to meet. Many organizations have received RFIs on one or both parts of the goal. It is important for organizations to move past simply obtaining the medication list and then returning it to the patient upon discharge.
This process, if implemented correctly, can prevent harm through partnering with patients on keeping accurate, up-to-date medication lists, reconciling medications to prevent unintended medication discrepancies, and actively managing patients’ medications as part of their larger treatment plan as patients move through the healthcare system.
