Hospitals scramble to address high risk drug errors

Hospitals across the country are implementing new policies and procedures for the prevention of errors when using high-risk medications, the Wall Street Journal reports.

According to the Institute for Safe Medication Practices, eight high-risk medications account for 31% of medication errors that harm patients.

In particular, hospitals are attempting to address the existing "grab and go" culture of healthcare providers dispensing medication without fully reading labels and other warnings.
