Drug mix-ups still increasing

The number of drug name mix-ups has doubled since 2004, according to a report by U.S. Pharmacopeia.

The organization, which regulates generic names for drugs and acts as an advisor to pharmaceutical companies, examined approximately 26,000 medical records and found that more than 1,400 unique drugs were involved in mix-ups with similarly named drugs, either brand named or generic.

According to the report, 519 healthcare facilities reported 176,409 medication mix-ups in 2006.
