Rhode Island Hospital has been fined $50,000 by the Rhode Island health department following the third incident on wrong-site surgery involving a patient's brain, according to an Associated Press report.

The 82-year-old patient did not experience a negative outcome from the wrong-site surgery, but a patient in one previous incident died as a result of the error. The hospital issued a statement that it was reexamining its training policies to prevent such errors from occurring again.
